Join Southlake’s New Extensivist Clinic Team – A Transformative Approach to Outpatient Care!
Are you passionate about caring for patients with complex medical needs and helping them stay healthy at home?
As a Nurse Practitioner, you will serve as the primary clinical lead for a panel of medically complex patients who are at high risk of emergency department visits, hospital admissions, and health deterioration. You will provide comprehensive assessment, diagnosis, treatment, and ongoing management of chronic and acute health conditions while working closely with physicians, nurses, allied health professionals, community partners, and caregivers.
This is an opportunity to practice to the full scope of your NP license, build meaningful longitudinal relationships with patients and families, and make a measurable impact on reducing avoidable hospital use through proactive, patient-centred care.
You will combine direct patient care with leadership, quality improvement, education, and program development activities to advance innovative models of community-based care.
About the role:
• Engage in direct and in-direct advanced clinical practice, education, research and consultation within the area of internal medicine to ensure the highest quality of care is provided to patients and families.
• Actively participate as part of the program and professional leadership team in promoting quality care by implementing evidence based practices, evaluating patient outcomes, development and evaluation of policies, clinical programs and services.
• Model both the professional and organization core competencies and provide expert clinical nursing leadership.
• Provide comprehensive specialty care focusing on health promotion, disease prevention, and chronic disease management. Working in collaboration with the team, the NP offers patient-centered care, including assessment, diagnosis, and treatment, while adhering to the top of their scope of practice.
What you’ll be doing:
Clinical Care and Patient Management
- Act as the primary clinical contact for a roster of medically complex patients with multiple chronic conditions.
- Conduct comprehensive health assessments, diagnose illnesses, order and interpret diagnostic tests, prescribe treatments, and monitor patient progress.
- Manage both acute health concerns and long-term chronic diseases, ensuring patients receive timely and appropriate care.
- Identify early signs of clinical deterioration and intervene promptly to prevent avoidable emergency department visits and hospital admissions.
- Provide rapid access and urgent clinical support for patients experiencing changes in their health status.
- Develop, implement, and adjust individualized care plans in collaboration with patients, families, caregivers, and the interdisciplinary team.
- Support patients transitioning between hospital, home, primary care, specialist care, and community services to ensure continuity of care.
Patient and Family Support
- Build trusted relationships with patients and caregivers through ongoing follow-up and longitudinal care.
- Educate patients and families about disease management, medications, treatment plans, and self-management strategies.
- Promote medication adherence and help patients navigate challenges related to treatment plans and symptom management.
- Connect patients with community resources and services that support their medical, functional, and social needs.
Team Collaboration and Leadership
- Collaborate closely with physicians, nurses, pharmacists, social workers, therapists, and community providers to deliver coordinated care.
- Participate in case conferences and care planning discussions for high-risk and complex patients.
- Provide clinical consultation and leadership to colleagues and partners across the care continuum.
- Champion evidence-informed practice and contribute to quality improvement initiatives that enhance patient outcomes.
Program Development and Quality Improvement
- Monitor patient outcomes and contribute to the evaluation of new care models, clinical pathways, and services.
- Participate in research, education, and program development activities that advance innovative community-based care.
- Support the development and implementation of policies, procedures, and best practices.
- Maintain accurate and timely documentation within the electronic medical record (EMR).
Why This Role Is Different
- Practice at the full scope of your Nurse Practitioner training.
- Build long-term therapeutic relationships with patients and families.
- Focus on proactive care rather than episodic treatment.
